Our Health Questionnaire Consultation is designed for those who would like a comprehensive appointment that will give direction and a detailed roadmap for your healing journey.
This session will help to guide you as we outline a possible treatment plan tailored specifically to your needs.

In our initial 45 minute analysis session session, you can expect to answer some simple questions about your health in our health questionnaire.
We will then begin a simple non-intrusive testing method which will give us a series of results.

The initial appointment will revolve around a detailed questionnaire that can be filled out in person or sent to you online.
We then schedule a 45 minute appointment to go through your options. This appointment can take place either in-person or via an online video appointment.
